How Botulinum Toxin Helps
When we inject a small amount of botulinum toxin directly into the masseter muscle, we are temporarily reducing that muscle's ability to generate its full force. We are not paralyzing it - you will still chew, still speak, still function completely normally. We are turning the volume down. Think of it like a muscle that has been sprinting for years without a rest day. The botulinum toxin gives it permission to stop. The clenching reduces. The grinding diminishes. The tension that has been building in the jaw, the temples, and the base of the skull begins to release. For many patients, the masseter gradually softens and reduces in size over subsequent treatments, producing a subtle but noticeable change in the jawline - a softer, less squared appearance. We want to be clear that this is a secondary consequence of the medical treatment, not the goal. We mention it because patients often notice it and want to understand why it is happening.
How Does It Work?
Assessment
Evaluation of jaw tension, clenching patterns, and masseter size.
Injection
A small number of carefully placed injections directly into the masseter muscle on each side. Takes about 10 to 15 minutes.
Recovery
No downtime. Avoid very hard or chewy foods for the first 24 hours.
Results

We want to make sure you have a complete and honest picture of how costs work for this treatment - because the insurance landscape can be genuinely confusing, and you deserve clarity before you book.
The Medication vs. The Treatment Fee
There are two separate costs associated with botulinum toxin treatment. The first is the cost of the medication itself - the actual drug. The second is the treatment administration fee - the physician's fee for the consultation, the injection technique, and the follow-up care. These are billed separately, and they are covered (or not covered) separately.
Private Drug Insurance: What May Be Possible
For TMJ pain and masseter hypertrophy, the botulinum toxin medication may be eligible for coverage under some private drug insurance plans - but this is not automatic, and it is not guaranteed. Every plan is different. Contact your insurer directly and ask whether botulinum toxin for TMJ is a covered benefit. Ask specifically whether a preauthorization is required before the medication will be considered for coverage.
If Coverage Is Declined
If your plan declines coverage - or if you do not have a private drug plan - you will be responsible for the medication cost privately. We offer lower-cost botulinum toxin options: Xeomin, for example, is often more affordable than Botox while delivering comparable clinical results. Dosing can sometimes be optimized to reduce the medication cost. We will always be transparent about costs before any treatment proceeds.
The Treatment Administration Fee
The physician's fee for administering the treatment - the consultation, the injection, and the follow-up - is a separate charge from the medication. In our experience, this fee is rarely covered by private insurance plans, regardless of the indication.
Medical Tax Deductions
Because TMJ is a medical condition - not a cosmetic concern - all costs associated with medically indicated botulinum toxin treatment - including the medication, the administration fee, and related receipts - may be eligible as a medical expense deduction on your Canadian tax return. This applies to the full treatment cost, not just the drug. Keep all your receipts. We will always provide proper documentation. We recommend speaking with your accountant or tax advisor to confirm eligibility based on your individual circumstances, but this is a meaningful benefit that many patients are not aware of.
A Note on Off-Label Use
Here is something important to understand about the TMJ and masseter pain application specifically: botulinum toxin is not officially approved by Health Canada for this indication. That means this use is considered 'off-label.' Off-label does not mean experimental or unsafe. It means the pharmaceutical company has not gone through the formal approval process for that specific use - often because the clinical evidence exists but the commercial case for pursuing a separate approval is not there. We use botulinum toxin for masseter pain because the evidence supports it and it works. Many well-established medical treatments are used off-label every day. The practical consequence for you, though, is real: because it is off-label, most private drug insurance plans will not cover the medication cost for this indication. In our experience, coverage for TMJ and masseter botulinum toxin is rare, and typically only arises through a Health Spending Account (HSA) - which is a type of employer benefit that allows you to spend a set dollar amount on health-related expenses with more flexibility than a traditional drug plan. If you have an HSA, this treatment may be eligible. We are always happy to provide the documentation you need to submit a claim.
Integrity in the Insurance Process
If your medication is approved and covered by your drug plan for a specific medical indication, that coverage applies exclusively to treatment of that condition - as documented, prescribed, and administered. Medication covered under a medical drug plan cannot, under any circumstances, be redirected toward cosmetic use. This would constitute insurance fraud, which carries serious legal and professional consequences for everyone involved. Similarly, preauthorization forms must be completed honestly and accurately. They are legal documents. Any misrepresentation of a patient's clinical situation - however well-intentioned - is not something we are able to participate in. We take the integrity of this process seriously. Our commitment is always to get you the coverage you legitimately qualify for, through a process that is completely above board.
What Is a Preauthorization Form?
A preauthorization form is a document your insurance company requires before they will approve coverage for certain medications. It outlines specific medical criteria - criteria the insurer has determined must be met for the medication to be considered medically necessary under your plan. Your physician completes the form based on your clinical history and current presentation. If your situation meets the insurer's criteria, coverage is approved. If it does not, coverage will be declined and you will be responsible for the medication cost privately. We are experienced with completing these forms accurately and efficiently.
